At LifeSpan, we LOVE our drivers, and our clients do too!  Due to an expanding program, LifeSpan is looking for a transportation driver to operate vehicles of the transportation program in a manner which ensures the safety of passengers. We take clients to medical appointments and social destinations. Drivers should have an excellent driving record, have a “Just for Hire” endorsement license and complete a background check and drug and alcohol screen. We offer flexible hours with a Monday-Friday schedule and a supportive and friendly work environment.  Case Manager

JOB TYPE: FULL TIME

Summary: LifeSpan Resources is the designated Area Agency on Aging and Aging and Disability Resource Center (ADRC) for Clark, Floyd, Harrison and Scott counties. The Case Manager will be trained to fulfill the agency's function of being the intake point for the Indiana Division of Aging's Home and Community Based Service programs, and includes other agency collaborations such as private pay initiatives, federal programs and care transition activities. The Case Manager ensures accurate, consistent, and efficient access to quality home and community based services.

 

Specifically: The Case Manager is responsible for educating, advocating and working with individuals of all ages, their caregivers and representatives to create person centered plans. Screening and assessment tools are used to discover individual's strengths, values, preferences and eligibility as required by various programs; with the goal of maintaining independence in the community. Case Managers may work in a variety of settings that include in-office, client home setting, hospitals and skilled nursing facilities.

 

Skills/ Abilities:

Excellent communication skills, both oral and written, with good interviewing skills. The ability to assess specific needs both in person and on the phone. Ability to empathize and understand individual behaviors. Knowledge of options, funding sources, services, resources and information available in the community.

 

Education Requirements:

A Bachelor's Degree in social work, gerontology, psychology, counseling or nursing; OR sociology with two years case management experience.

OR A registered nurse currently licensed to practice in Indiana with one-year experience in a human service setting; OR

Bachelor's Degree in any other field with a minimum of two years full-time, direct service experience with the elderly or person with disabilities. This experience must include assessment, care plan development and monitoring; OR

Master's Degree in a related field may substitute for the required experience.
