to persons 60 years of age or older and persons
with disabilities in Clark, Floyd, Harrison and
Scott counties.

WHAT IS THE INFORMATION & ASSISTANCE PROGRAM?
• This Program provides seniors and persons with disabilities options and
answers to questions about In-Home and community based services. These services
help persons remain in their own home as long as possible. Some of the services
are care management, respite care, attendant care, homemaker, home delivered
meals, adult day services, home health aide and transportation.
• The
I& A Program provides assistance to callers
by making referrals to other social service agencies,
home health agencies, and government organizations.
The I&A Coordinator can make contacts on behalf
of a client to help them understand a process,
problem solve or advocate for them.
• The
Information & Assistance Coordinator provides
brochures, pamphlets and resource lists to clients
on most subjects pertaining to issues and concerns
of older adults and persons with disabilities.
These resources will help client’s access
local, state and federal programs. Resource information
is available on the following information and can
be mailed to you per your request.

MINORITY
OUTREACH
The Information & Assistance Coordinator provides information to minority
individuals/groups about current resources, services, and job opportunities.
The I& A Coordinator is available to give presentations on various topics
to groups at their request.

The I&A program is the entry point to receive in-home services and community
